If you would like to make a claim against any of the above benefits either contact your Welfare Society Station Representative or fill out the Benefit Application (BA1)  form with copies of original receipts attached.
In the case of a Birth Of A Child Benefit a birth notice from the paper or a copy of the birth certificate must be attached. 
To claim the Funeral Assistance Benefit a copy of the death certificate or the death notice must be provided.

The Cost? - Only $3.00 per Week.
